Looks like one of the couple hundred SimpleCraft sort of packs. The grass/dirt looks especially familiar, but I don't remember what it's from

At first glance I thought it was Squareswept by Leostereo but upon closer analysis it turns out it wasn't.
If you can't find the one you're looking for I'd recommend this one as it's fairly similar in style
If the minecraft world is infinite, why does the sun still rotate around it?

That screenshot's from the Dota map isn't it?
All parts should go together without forcing. You must remember that the parts you are reassembling were disassembled by you. Therefore, if you can't get them together again, there must be a reason. By all means, do not use a hammer.
