I want to use it for something and would like to get the original artist's permission.

I dunno. Sounds like it would be a derivative work, I think you'd be fine. Though I personally think 'photoshopping a penis' should have its own clause under fair use, just to be sure.

On topic, though, yeah. I believe he gave permission to most people to use it as they saw fit back in the day. I'd have to double check, but I'm quite sure he'd be fine with the owner of the mod using the banner for... what I would assume isn't just a lolly screw.
